ONC Publishes USCDI v7 With 31 New Health Data Elements to Strengthen Telehealth Interoperability 

WASHINGTON, D.C., United States — July 24, 2026 — The U.S. Office of the National Coordinator for Health Information Technology (ONC) released the United States Core Data for Interoperability Version 7 (USCDI v7) on July 23, 2026, introducing 31 new and revised standardized health data elements designed to advance nationwide electronic health information exchange — a development with direct and material implications for the expanding telemedicine market, which was valued at USD 141.13 billion in 2024 and is projected to reach USD 399.75 billion by 2030 at a CAGR of 19.0%. 

Main Development 

Published through ONC Standards Bulletin 2026-2 (SB26-2), USCDI v7 represents the most comprehensive annual update to the federal interoperability data standard in recent years. The release adds 30 new data elements and one significantly revised data element — Tobacco and Nicotine Product Use, an evolution of the prior Smoking Status element — spanning multiple clinical data classes. 

Among the additions most consequential to virtual care delivery are the Appointment data element, which standardizes the exchange of scheduled healthcare event information including date, time, location, and provider details; Referral Order and Referral Note, which together strengthen care coordination across specialties and organizations; and Medical Device Order, which standardizes provider-authored requests for devices such as home blood pressure monitors and infusion pumps — a category central to remote patient monitoring (RPM) workflows. 

The Medication Administration data element, which documents structured information about medications administered to a patient, is also included, supporting medication reconciliation and care transitions in virtual care settings. Additionally, Healthcare Agent — identifying the person legally authorized to make healthcare decisions on behalf of a patient — has been added to support advance care planning and timely decision-making during care transitions, including those conducted remotely. 

ONC noted that 15 of the 31 new data elements are already represented in implementation specifications required under the ONC Health IT Certification Program and are largely supported by certified health IT systems. Their formal inclusion in USCDI v7 reflects ONC's continued effort to align the standard with interoperability capabilities already widely deployed across the healthcare ecosystem. 

The USCDI standard is mandated across multiple federal programs, including the Centers for Medicare & Medicaid Services' Interoperability and Prior Authorization Final Rule (CMS-0057-F) and the Trusted Exchange Framework and Common Agreement (TEFCA), ensuring that USCDI v7 will propagate across payers, providers, and health IT platforms operating within federal healthcare programs. 

With the publication of USCDI v7, ONC has simultaneously opened the submission period for USCDI v8 through the ONC New Data Element and Class (ONDEC) system. The public comment period closes on Monday, September 28, 2026, at 11:59 PM ET, with Draft USCDI v8 targeted for release in January 2027.
